Criminal Convictions

Rehabilitation of Offenders Act

We are asking you to answer the below in relation to job roles which are not exempt from the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (2013 and 2020). For this reason you are only required to disclose information about unspent convictions. You are not required to disclose spent convictions on this form. We will seek to put forward/supply the best possible candidates to our clients. Having a criminal conviction will not necessarily exclude you from the process. The information given will be treated in the strictest of confidence and only taken into account where, in the reasonable opinion of Capital Outsourcing Group, the offence is relevant to the post to which you are applying. Failure to declare a conviction may require us to exclude you from our register to terminate an assignment if the offence is not declared but later comes to light.

HEALTH SELF-ASSESSMENT DECLARATION FOR NIGHT WORKERS:

In compliance with the Working Time Regulations, the purpose of this declaration is to assess your fitness to carry out night work whilst on an assignment with Capital Outsourcing Group Limited. While, there is no difference in the physical demands of night work compared to daytime work, the human body is naturally programmed to sleep at night. The disruption to this sleep pattern can aggravate certain medical conditions, albeit often temporarily. Additionally the reduced access to services and facilities that may be apparent both inside and outside of the workplace may also have an effect.

However, should your answers raise concerns about your fitness for night work, then unfortunately you may not be assigned to organisation’s requiring night working, unless you are able to provide proof that you are indeed physically able to do so having been assessed by a qualified healthcare or medical professional.
